Answer : When there is a bug in the single player game.
I friend of mine has just completed the single player on what he thought was realistc but didn't get the achievement. On searching the net it would appear that there is a bug in the game, where if you start the game on normal then start a new game on realistic, when you load a check point it will revert back to normal difficulty.
I checked my games last night because when I first got it I gave the single player a quick try on normal then statrted properly on realistc and sure enough it had reverted back to normal. The problem is that, as far as I know there is no way of telling what levels have been completed on which difficulty. It would appear that the only way to get around this is to completely delete your save and start again :mad:
Has this happened to anyone else?
